Post Hospitalization Care

LEVEL 1 — TRANSITION HOME

Designed for: Clients who are medically stable and primarily need professional nursing oversight during the first days home after hospitalization.

Services Include:

• Review of hospital discharge instructions

• Medication reconciliation

• Medication schedule review and education

• Vital signs monitoring

• Wound/ Incision Assessment and care

• General health and recovery assessment

• Review of symptoms and potential warning signs

• Assessment of pain and comfort

• Review of nutrition and hydration needs

• Basic mobility and fall-risk assessment

• Review of home safety concerns

• Reinforcement of physician and discharge instructions

• Coordination with the client's physician or healthcare providers as needed

• Identification of changes in condition requiring medical attention

• Communication with designated family member or caregiver

The goal: Help the client transition safely from hospital to home while giving the family confidence that someone with professional nursing experience is paying attention to the details.

This is a one time visit that takes approx 90 minutes.
